Service Consent

I understand that MAI TAI Home Massage Services provides professional wellness massage for relaxation, stress reduction, muscle tension relief, and general wellbeing only. The service is not medical treatment and is not intended to diagnose, treat, or cure any medical condition, illness, or injury. MAI TAI therapists do not provide medical advice, diagnosis, physiotherapy, or medical treatment. I agree to consult a licensed doctor before booking or receiving a massage if I have any medical concerns.

I confirm that I have disclosed all relevant health conditions, injuries, allergies, medications, pregnancy status, or other information that may affect the massage. I accept full responsibility for my health condition and for choosing to proceed. If I experience pain, discomfort, dizziness, or any unusual reaction during the massage, I will immediately inform the therapist so the pressure, technique, or session can be adjusted or stopped.

Health Conditions

Please inform MAI TAI before your appointment if you have any condition or concern, including high or low blood pressure, heart conditions, allergies, injuries, recent surgery, pregnancy, skin conditions or infections, fever, flu, contagious illness, chronic medical conditions, or medication that may affect your treatment. MAI TAI may refuse or stop the service if the therapist believes the massage may not be safe for the customer.

Professional Conduct, Privacy, and Draping

MAI TAI is strictly a professional wellness service. I agree not to engage in inappropriate behavior, including sexual comments or advances, harassment, physical or verbal abuse, threatening behavior, intoxication, drug use, or any conduct that makes the therapist feel unsafe or uncomfortable. The therapist may immediately stop the service and leave; the booking may be charged in full, and MAI TAI may refuse future bookings.

Professional towel draping will be used to respect privacy and comfort. Only the body area being worked on will be uncovered. Underwear must be worn at all times throughout the massage.

Appointment Timing

Please be available and ready at the confirmed appointment time. If you are late or not ready when the therapist arrives, inform us immediately by phone or WhatsApp. We will do our best to accommodate you; however, the massage may still need to end at the original scheduled finish time. Customer delays may reduce the massage duration without refund or price adjustment. If the therapist is delayed due to traffic, weather, building access, or unforeseen circumstances, MAI TAI will inform the customer and will do its best to provide the full booked duration, subject to schedule availability.

Home Service Requirements

The customer must provide a safe, clean, private, and suitable environment, ensure safe access to the location, keep pets away if needed, not be under the influence of alcohol or drugs, and provide building access, parking, or security instructions in advance. MAI TAI may cancel the service if the location is unsafe, inappropriate, or unsuitable for the therapist.

Cancellation and Rescheduling

Once confirmed, MAI TAI reserves a therapist specifically for the appointment. Please notify us as early as possible for cancellation or rescheduling. Requests made less than 4 hours before the appointment may be subject to a cancellation fee. Failure to provide sufficient notice, failure to answer when the therapist arrives, incorrect location details, or refusal of service upon arrival may result in a charge of 50% of the booking amount or a minimum fee of 100 AED, whichever applies.

Customer Acknowledgment

By booking and receiving MAI TAI Home Massage Services, I confirm that I have read, understood, and agreed to this consent form; disclosed any health conditions that may affect the massage; understand that the service is for wellness and relaxation only; and agree to maintain respectful and appropriate conduct at all times.
